Job Description: The RN Case Manager ensures appropriate utilization of resources, facilities, and services by the review of admissions, duration of stay, professional services rendered. For both inpatients and observation patients. Identifies patient discharge needs and works in collaboration with nurses and physicians in managing the patients care needs through the organization. Maintains completed and up to date documentation of patient discharge plans and collaborates with all disciplines in documenting patient needs. Updates the discharge plan based on the patient needs and responses to treatment.
