Case Manager - NURSE: CASE MANAGEMENT

Job Summary:
The RN Case Manager ensures appropriate utilization of resources, facilities, and services by the review of admissions, duration of stay, professional services rendered. For both inpatients and observation patients. Identifies patient discharge needs and works in collaboration with nurses and physicians in managing the patients care needs through the organization. Maintains completed and up to date documentation of patient discharge plans and collaborates with all disciplines in documenting patient needs. Updates the discharge plan based on the patient needs and responses to treatment.

Requirements:  

         Current Florida RN license or Foreign Medical Graduate required

         Minimum 3-5 years experience as a case manager in an acute care setting (hospital) required

         Lead case management experience preferred, familiar with and proficient at applying and documenting InterQual criteria

         Familiar and proficient at KePRO

         Previous RN experience in a variety of provider clinical settings preferred but not mandatory

         Candidate must have strong communication and presentation skills, demonstrated self-motivation
